Watch: HRT and Breast Cancer | What the Science Really Says!
Breast cancer — estrogen alone. I mentioned this last time but it deserves its own moment. In the WHI arm where women had a hysterectomy and took estrogen by itself, breast cancer went down. About twenty-three percent lower. And in the long-term follow- up, breast cancer deaths were lower too. That’s not me spinning it, that’s the same study everyone weaponizes. The signal in the other arm came from the synthetic progestin — not from estrogen.
Endometrial cancer. This one’s nuanced so listen carefully. Estrogen by itself, unopposed, in a woman who still has a uterus — that absolutely raises endometrial cancer risk, and that’s real, that’s not controversial. But that’s exactly why we don’t do that. When you add progesterone properly, that risk gets neutralized. And here’s the interesting part — with continuous combined therapy, the data suggests risk may actually come out slightly lower than not treating at all. Progesterone matures the lining and keeps it from building up. So the “estrogen causes uterine cancer” line is true only when it’s prescribed badly.
And honestly? The biggest driver of endometrial cancer in this country isn’t a prescription. It’s obesity and insulin resistance — belly fat making estrogen with no progesterone to balance it, year after year. That’s unopposed estrogen too. Just the homemade kind.
Now let me be straight with you, because I won’t do the thing where I only show you one side.
Ovarian cancer risk does appear to go up slightly on HRT — about one extra case per thousand women over five years. Small, but real. And in the WHI, the estrogen-plus- progestin arm showed more deaths from lung cancer, particularly in smokers. So this isn’t a free lunch. It’s a ledger. And when you actually add up that ledger — colon down, breast down or flat with the right progesterone, bone fractures cut in half, and started early, lower all-cause mortality — the ledger comes out in your favor for most women.
That’s the whole point. Not “hormones are safe.” Not “hormones are dangerous.” Hormones are a decision with a real balance sheet, and for most women who start within ten years of menopause, that balance sheet is good. You deserve to see both columns.
The problem is nobody ever showed you the second one.
